Abstract
Background: Oral dysbiosis contributes to dental caries and periodontal disease. This
randomized, double-blind, placebo-controlled trial evaluated the effects of heat-killed Lactobacillus
paracasei PE0301 on oral microbiota and immune markers. Methods: 150 adults with moderate caries risk
received PE0301 lozenges (2×10¹⁰ cells/day) or placebo for 8 weeks. Primary outcomes included Streptococcus
mutans counts and salivary IgA levels. Secondary outcomes assessed plaque index, gingival health, and
microbiome composition. Results: PE0301 group showed 42% reduction in S. mutans counts (p<0.001) and 34%
increase in salivary IgA (p<0.001). Plaque index decreased by 28% and gingival bleeding reduced by 31%.
16S rRNA sequencing revealed increased beneficial Lactobacillus and Bifidobacterium species. Conclusion:
PE0301 effectively modulates oral microbiota and enhances local immunity, supporting oral health
maintenance.
